The bracket visible near the corner of the heatshield would hold the Service Module/Spacecraft-Lunar Module Adapter umbilical lines, which would run to the white, rectangular cut-out on the heatshield base.

The notch in the heatshield base and the holes in the SM aft face is where the Unified S-Band high gain antenna would attach.
