The second stage appears to be topped with a standard Mark 6 reentry vehicle. See my photos of the Mk. 6 RV at the 8th Air Force Museum, the Strategic Air & Space Museum, and Stafford Air & Space.

Referring to my Titan II Missile Configuration diagram, we see that the second stage is displayed with its interstage structure.
