Although the first several lines are obscured, the tag reads

Since Borman's Apollo 8 CWG has a serial number of 1116 but this CWG appears to have a serial number of 1131, I would guess that it's from Apollo 13, rather than Apollo 8. Additionally, the tag on Borman's Apollo 8 CWG has a date on it, but Lovell's does not, but the contractor may have changed the tag design in the intervening years.
